Frequently Asked Questions

Getting Started
Can I take my Fem Excel treatment plan to my Primary Care Provider (PCP)?
You are welcome to share your plan, but most PCPs are not trained in hormone optimization or the use of bioidentical hormones, so they may not feel comfortable continuing or managing your treatment. For the safest and most effective results, we recommend continuing your care within the Fem Excel program.
How do I know if my symptoms are hormone-related or something else?
Many issues like fatigue, weight gain, poor sleep, or mood swings can be hormone-related. The best way to know is through a combination of symptom review and lab testing. Every Fem Excel membership includes a consult with a hormone specialist and an at-home test kit.
Do I have to be on hormones forever?
Not necessarily. Many women stay on therapy long term because of the benefits, but your plan is always personalized and can be adjusted over time with your provider.
How do Fem Excel providers receive their training?
Every provider completes in-house training developed by Dr. Peter Fotinos and Dr. Lorna Brudie. Training covers evidence-based bioidentical hormone therapy, symptom-based dosing, thyroid optimization, safety monitoring, and peer case reviews. Providers also complete ongoing continuing education.
Are Fem Excel providers part of a larger doctor network?
No. Fem Excel providers are part of our own dedicated medical team, not an outsourced or rented network. Every clinician works under Fem Excel medical leadership and follows The Excel Advantage protocol.
How do I order my at-home hormone test kit?
- Take the free assessment.
- Have a virtual consult with a hormone expert. If prescribed, treatment can begin immediately.
- Receive your at-home lab kit.
- Your provider reviews your results and adjusts your plan for the best outcome.
Why do I need a hormone test?
Our at-home test measures key biomarkers, including estrogen, progesterone, testosterone, thyroid function, and cortisol. These results help your provider personalize your plan and track progress over time.
How accurate are at-home hormone tests?
Fem Excel uses CLIA-certified laboratory testing that meets clinical standards. Licensed providers interpret results alongside your symptoms and medical history.
Is the Tasso+ at-home blood collection device FDA-approved?
Yes. Tasso+ is FDA-approved and designed for safe, comfortable, and reliable at-home blood collection.
Can I start treatment before I get my test results?
Yes. At Fem Excel, treatment is symptom-led rather than solely based on lab results. If your symptoms clearly indicate a hormone imbalance, your provider may prescribe your treatment after your consultation, without waiting for test results. Once your lab results are available, your provider will use them to confirm and fine-tune your plan.
How often do I need to test my hormones?
Typically, every six months. Follow-up test kits are included in your membership.
Treatment
How long does it take to see results?
Many women notice more energy, better sleep, and improved mood within four to six weeks. Full optimization, including changes in weight, strength, and body composition, can take four to six months or up to a year.
What hormones do you prescribe?
Depending on your hormonal profile and any conditions such as PCOS, your plan may include bioidentical estrogen, progesterone, testosterone, and thyroid medication. Your provider may also recommend additional medications to support your goals.
Do you prescribe testosterone for women?
Yes. Testosterone supports libido, muscle mass, energy, and cognitive function. We monitor levels to keep them optimized for maximum benefits.
How does thyroid medication fit into my treatment plan?
If symptoms and history suggest low thyroid function, your provider may include thyroid hormone replacement to improve energy, metabolism, and mental clarity.
Will testosterone cause masculine side effects such as facial hair or deepening of the voice?
No. When given at the correct dose for women and monitored by a Fem Excel provider, testosterone does not cause masculine changes. Your treatment is personalized and reviewed regularly to keep your levels optimal.
How does Fem Excel ensure treatment safety?
Every plan is medically supervised. Treatment is guided by your symptoms first, with blood tests used to support decisions when needed. You will complete a symptoms assessment every 60 days, and your provider will review your progress with scheduled follow-ups every six months, with additional check-ins if required.
How often will my provider follow up?
Your provider will review your progress every 60 days through your symptom assessment and adjust your plan if needed. You will also have scheduled follow-ups every six months, with additional check-ins if required. You can contact your provider through the member portal between reviews if you need support.
Which labs do you check as part of treatment?
Symptoms guide treatment first, with labs used to support decisions when needed. We monitor different biomarkers depending on whether you are pre, peri, or post-menopause, or have PCOS. This can include estrogen, progesterone, testosterone, thyroid markers (TSH, Free T3, Free T4), and, when appropriate, insulin, vitamin D, lipid profile, and other metabolic markers. Up to 13 biomarkers are reviewed over time to keep your plan aligned with your needs.
Are there side effects when starting hormone therapy?
Some women may experience temporary changes, such as mild skin sensitivity, sleep shifts, or breast tenderness in the early stages. These are usually short-lived and part of the body adjusting. Your provider will monitor you and make adjustments if needed.
Who should not take hormone therapy?
Women who are pregnant, breastfeeding, or have certain medical conditions may not be candidates for hormone therapy. Your provider will review your health history to determine if treatment is right for you.
Can I use Fem Excel if I have a history of breast cancer?
Your provider will review your specific history to determine if hormone therapy is appropriate. In some cases it may not be recommended. We encourage you to share all relevant medical details during your consultation.
Does Fem Excel offer guidance on nutrition or exercise?
Yes. Providers can offer practical guidance on nutrition, exercise, and daily habits that support optimal hormone function and enhance results.
Will I gain weight when starting HRT?
Bioidentical hormone therapy is not associated with weight gain. Many women find that as hormones become more optimal, managing weight and improving body composition becomes easier. You may notice changes in how clothes fit and how your body feels before seeing the scale move, as muscle is denser than fat. Individual results vary, and many women say they finally feel like their body is working with them again.
Do I need to change my diet or lifestyle during treatment?
Healthy habits can support optimal results. While medication plays a key role, nutrition, movement, and sleep also influence how you feel. Your provider can offer simple tools that fit your routine.
Can I combine Fem Excel treatment with supplements?
Yes, but we recommend sharing any supplements you take with your provider so they can ensure everything works well together and supports your treatment goals.
Hormones
What is the difference between bioidentical and synthetic hormones?
Bioidentical hormones have the same molecular structure as your natural hormones and are typically better tolerated. Synthetic hormones are chemically different and may cause unwanted effects. Fem Excel prescribes only bioidentical hormones compounded to pharmaceutical grade standards.
Where are Fem Excel medications made?
In U.S.-based, FDA-registered compounding pharmacies that meet strict safety and quality standards. Prescriptions are tailored to your provider’s specifications.
Is hormone therapy safe for menopause?
Yes. Hormone therapy is considered safe for most women when medically supervised. It can improve quality of life, symptoms, and long-term health when prescribed by a trained provider. Your Fem Excel provider will review your health history to confirm if treatment is suitable for you.
Can I take HRT if I am still having periods?
Yes. Many women begin treatment during perimenopause before their periods stop. Your provider will tailor your plan to your symptoms and cycle changes.
Can I take HRT after a hysterectomy?
Yes. Women who have had a hysterectomy can still benefit from hormone therapy. Your provider will recommend the appropriate combination based on whether your ovaries were removed.
PCOS
What is PCOS?
Polycystic Ovary Syndrome is a common hormonal disorder. Symptoms can include irregular periods, acne, weight gain, excess hair, and fertility issues, with higher risks of insulin resistance and type 2 diabetes.
How is PCOS diagnosed?
Diagnosis may include medical history, physical exam, blood tests to check hormone levels, and a pelvic ultrasound to assess the ovaries.
How does Fem Excel help women with PCOS?
We address root causes such as insulin resistance and hormonal issues with personalized hormone optimization, thyroid support, and lifestyle guidance.
What treatments are available for PCOS at Fem Excel?
Bioidentical hormone therapy, thyroid optimization, nutrition and activity support, and, when appropriate, medications such as metformin or semaglutide for weight management.
Can I lose weight if I have PCOS?
Yes. Hormone optimization plus nutrition and activity support, and when appropriate, metformin or semaglutide, can help regulate metabolism and support healthy weight management.
How much does Fem Excel PCOS treatment cost?
Membership is $99 per month. Medication starts at $50 per month and can vary by plan.
Is Fem Excel’s plan right for me?

However, you may be able to use your FSA, HSA, or HRA for reimbursement. Need more details? Call Member Support at (833) 625-3392 Monday to Sunday, 8 AM to 8 PM EST.

When will I be charged for my treatment?
Membership is billed monthly on your billing date. Medication costs are billed every two months based on your prescription plan.
What is the Fem Excel Guarantee?
If you do not see results or feel better within 90 days of starting treatment, we will refund your membership fee. This is our commitment to real, measurable progress.
How do I request a refund under the Guarantee?
If you have completed your prescribed plan for 90 days without improvement, contact Member Support at (833) 625-3392 or message us in your portal. A team member will review your case and process your refund promptly.
